If you are an LRI (legally responsible individual) and you are taking care of a spouse or a child under the age of 18, you will click the link and fill out the SFC application. If you are servicing someone under attendant care or just applying in general for employment you will click the link and hit the tab for attendant care employment. You will also be able to upload all required documents and go to the link to schedule your background check if you do not have a current one from the last 3 months.
